About Hope Health Network

Our Mission

Hope Health Network exists to ensure that every person we serve, in the Charleston Lowcountry and across South Carolina, has access to quality, coordinated healthcare regardless of their situation. We connect individuals to the right practice for their specific needs, whether that is primary care, behavioral health, or substance use treatment. Our network model means patients are never told "we do not handle that" — they are routed to the right provider within our system.

Four Practices, One Care Team

Hope Health Network is built around patient choice. Some patients want everything in one place: their primary care provider, their psychiatric prescriber, and their addiction medicine team sharing one chart and coordinating directly. Others have providers they trust and need one focused practice that integrates cleanly with their existing care team. We accommodate both, and with your consent every practice works from the same chart.

Hope Medical Associates provides comprehensive primary care for adults — annual physicals, chronic disease management, preventive care, and specialist referrals.

Hope Behavioral Medicine provides psychiatric medication management and counseling for anxiety, depression, ADHD, and insomnia — in person and via telehealth statewide across South Carolina.

Hope Integrative Medicine brings primary care, mental health, and addiction medicine under one roof for patients who want one chart and one coordinating team.

Hope Addiction Medicine provides medication-assisted treatment for opioid and alcohol use disorders — Sublocade injections, buprenorphine management, and naltrexone.

Separate at the front door, connected at the back door. Each practice has its own clinical focus, its own providers, and its own identity — but our patients benefit from seamless internal referral pathways. A patient at Hope Integrative Medicine who needs a primary care provider is referred to Hope Medical Associates. A patient at Hope Medical Associates who needs psychiatric evaluation is referred to Hope Behavioral Medicine. The coordination happens internally — the patient does not have to navigate the system alone.

Our Approach to Community Health

We do not just treat patients who walk through the door. We partner with courts, hospitals, social services, faith communities, employers, and housing organizations to reach individuals who might not seek care on their own. Our community programs describe these partnerships in detail.

Leadership

Hope Health Network is managed by Corporate Management Partners, LLC (CMP), a management services organization based in Charleston, SC. CMP brings deep expertise in healthcare technology, clinical operations, and practice strategy — ensuring that our clinical teams can focus on patient care while the operational infrastructure runs efficiently behind the scenes.
