Who We Serve

Hope Health Network works with drug courts, probation offices, pretrial intervention programs, and defense attorneys across the Charleston Lowcountry to connect justice-involved individuals to the medical and behavioral health treatment they need. Whether a patient has been ordered into treatment by a judge, referred by a probation officer, or connected through a defense mitigation team, we provide a clear pathway to care — and the compliance documentation the court requires.

Many individuals in the justice system have co-occurring substance use disorders, untreated mental health conditions, and unmet primary care needs. Our network of four specialized practices means we can address all of these needs under one roof, with coordinated treatment planning and a single point of contact for the referring agency.

Substance Use Treatment

Hope Addiction Medicine provides medication-assisted treatment (MAT) for opioid and alcohol use disorders. MAT is the gold standard for opioid use disorder — it reduces relapse, overdose death, and criminal recidivism. Our providers prescribe buprenorphine (Suboxone), Sublocade monthly injection, and naltrexone (Vivitrol). We monitor compliance through regular urine drug screens and provide court compliance reports on the schedule your court requires — weekly, biweekly, or monthly.

Behavioral Health Treatment

Hope Behavioral Medicine provides psychiatric medication management and counseling for co-occurring conditions including anxiety, depression, ADHD, PTSD, and insomnia. Many justice-involved individuals have untreated mental health conditions that contribute to substance use and recidivism. Our psychiatric providers evaluate, diagnose, and prescribe — and our licensed therapists provide individual counseling. Treatment documentation is provided to the court upon patient authorization.

Primary Care Establishment

Hope Medical Associates provides primary care for individuals who have never had a regular doctor or who lost access to care during incarceration. Establishing a medical home is one of the strongest predictors of successful re-integration. Our primary care providers manage chronic conditions (diabetes, hypertension, COPD), perform wellness exams, order labs, coordinate specialist referrals, and serve as the patient's long-term medical home.

Compliance Reporting

We understand that courts need documentation. Our compliance reports include attendance verification (dates and times of all appointments), urine drug screen results (with chain-of-custody when required), medication compliance (prescription fill history, pill counts, Sublocade injection dates), treatment plan adherence, and provider notes. Reports are faxed or mailed on the schedule your court requires. We respond to verification calls same-day when possible.
