Entity Structure

Hope Health Network is a referral coordination hub managed by Corporate Management Partners, LLC. Clinical services are delivered by SC Professional Services LLC (d/b/a Hope Integrative Medicine, d/b/a Hope Behavioral Medicine, d/b/a Hope Medical Associates) and SC Counseling Services LLC (d/b/a Hope Counseling). Each clinical entity maintains its own Notice of Privacy Practices. Contact the specific practice for their HIPAA notice.

Referral Information

When you refer a patient to Hope Health Network, the referral information you provide (patient name, contact information, reason for referral) is shared with the appropriate clinical practice for scheduling and intake purposes. This information is handled in accordance with each practice's HIPAA policies.
