Confidential, compassionate referral pathways for congregants who need medical or behavioral health care.

Why Faith Communities Matter

Pastors, ministers, rabbis, imams, and faith leaders are often the first people congregants turn to when struggling — with health problems, mental health concerns, family crises, or substance use. Yet faith leaders are not medical providers and often do not know where to refer. Hope Health Network provides a clear, confidential pathway from the pew to the provider.

We respect the role of faith in our patients' lives. For many, their faith community is their strongest support system. Our goal is to complement that support with professional medical and behavioral health care — not to replace it.

What We Offer Faith Partners

A single phone number that routes to the right practice. The congregant calls (843) 737-0312, we triage, we schedule. No forms to fill out, no medical knowledge required from the faith leader. The referral can be as simple as telling your congregant to call Hope Health Network and mention that their pastor suggested they reach out. We handle everything from there.

Primary Care

Hope Medical Associates provides primary care for congregants who need a medical home — especially older adults, those without insurance history, and those who have avoided the doctor for years. We accept Medicare, most major commercial plans, and all South Carolina Medicaid managed care plans except Select Health (First Choice). Annual wellness visits, chronic disease management, and preventive care are all available.

Behavioral Health

Hope Behavioral Medicine provides psychiatric medication management and counseling for anxiety, depression, grief, PTSD, and life transitions. Seeking professional help is not a failure of faith — it is an act of stewardship over the body and mind. Telehealth is available statewide for congregants who cannot travel to our West Ashley office.

Substance Use Treatment

Hope Addiction Medicine provides confidential medication-assisted treatment for opioid and alcohol use disorders. We understand that stigma within faith communities can make it especially difficult to seek help for substance use. Our treatment is confidential, non-judgmental, and evidence-based. Federal regulations (42 CFR Part 2) provide additional confidentiality protections for substance use treatment records.

For Faith Leaders

You do not need to diagnose or assess — simply provide our phone number when you sense a congregant is struggling. We also offer brief educational sessions for your ministry team on recognizing when someone might benefit from professional care, how to make the referral compassionately, and what confidentiality protections are in place.
