Re-Entry Programs

Medical home establishment for individuals transitioning from incarceration back into the Charleston Lowcountry community.

Who We Serve

Hope Health Network provides healthcare services for individuals released from county jail, state prison, or federal facilities who need to establish or re-establish medical care in the Charleston Lowcountry. The first 30 days after release are the highest-risk period for overdose death among individuals with opioid use disorder — having a medical home and MAT provider in place before or immediately after release saves lives.

We work with re-entry coordinators, parole officers, halfway house managers, and community organizations to ensure that returning citizens have a clear pathway to care from the day they walk out.

MAT Continuation

Hope Addiction Medicine is often able to schedule MAT continuation appointments within a week of release to help avoid a gap in medication. Many incarcerated individuals were on buprenorphine or naltrexone during incarceration or initiated MAT as part of a jail-based program. We continue that treatment without interruption. Buprenorphine bridge prescriptions are available to prevent gaps between release and first appointment. We also initiate MAT for individuals who were not treated during incarceration but need it upon release.

Behavioral Health Stabilization

Hope Behavioral Medicine provides psychiatric medication continuation for individuals who were stabilized on psychotropic medications during incarceration. Anxiety, depression, and PTSD are highly prevalent in re-entry populations. We are often able to schedule psychiatric follow-up within a week of release to continue medication management and prevent destabilization. Our licensed therapists also provide individual counseling to support the transition.

Primary Care Establishment

Hope Medical Associates establishes a medical home for re-entering individuals — many of whom have never had a regular doctor. We address deferred medical needs including chronic disease management, hepatitis C screening, HIV testing, dental referrals, and general wellness. Having a PCP is one of the strongest predictors of successful community re-integration and reduced recidivism.

If you are a re-entry coordinator, parole officer, or halfway house manager, call (843) 737-0312 and ask for our referral coordinator. We can schedule pre-release appointments for individuals with known release dates, ensuring care is available from day one. We accept Medicare, most major commercial plans, and all South Carolina Medicaid managed care plans except Select Health (First Choice). Self-pay is also available.
