The Charleston Lowcountry has a growing population of adults aged 65 and older. Many seniors struggle to find a primary care provider accepting new Medicare patients. Others have complex medical needs requiring coordinated care across multiple conditions. Hope Health Network provides a medical home where seniors receive unhurried, comprehensive care from providers who take the time to listen.

Our network's four specialized practices mean that seniors with overlapping needs — chronic disease management plus behavioral health support, for example — receive coordinated care under one roof rather than fragmented care across multiple unconnected offices.

Medicare Annual Wellness Visits

Hope Medical Associates conducts thorough Medicare Annual Wellness Visits covered at zero cost under Medicare Part B. Our AWVs include a comprehensive health risk assessment, personalized prevention plan, screening schedule review, advance care planning discussion, and immunization review. Our providers conduct thorough evaluations — not the brief, check-the-box visits common elsewhere. These visits are an opportunity to catch emerging health issues early and update your care plan for the year ahead.

Chronic Disease Management

Hope Medical Associates manages the chronic conditions that are more prevalent and more complex in older adults — diabetes, hypertension, heart disease, COPD, and thyroid disorders. Regular follow-up visits, medication optimization with careful attention to polypharmacy risks in seniors, lab monitoring, and specialist coordination ensure that each condition is managed as part of a cohesive treatment plan rather than in isolation.

Behavioral Health for Older Adults

Hope Behavioral Medicine provides psychiatric medication management and counseling for late-life anxiety, depression, insomnia, and adjustment disorders related to retirement, loss, health changes, and social isolation. Our psychiatric providers understand the unique medication considerations for older adults — drug interactions with existing medications, altered metabolism, and fall risk from certain psychotropic medications. Telehealth is available for homebound seniors or those with transportation challenges.

We partner with the Area Agency on Aging — Trident, Meals on Wheels, home health agencies, assisted living facilities, independent living communities, and senior centers across the Lowcountry.
