Call (843) 737-0312 and ask for referral intake. Initial evaluations are often able to be scheduled within a week of the request.

By Fax

Fax referral documentation to (843) 279-3161. Include patient name, insurance, reason for referral, and any relevant clinical notes. We typically contact the patient within two business days.
