Who We Serve
Hope Health Network provides healthcare services for individuals currently in emergency shelters, transitional housing, Oxford Houses, sober living facilities, or recently housed after a period of homelessness. Establishing a medical home is one of the most stabilizing steps a person can take during a housing transition — it creates continuity, addresses deferred health needs, and provides a foundation for long-term stability.
We work with shelter staff, housing coordinators, and case managers to ensure that residents have a clear pathway to care from the moment they enter a program.
Primary Care
Hope Medical Associates establishes a medical home for individuals who may not have seen a doctor in years. Deferred medical needs are common — untreated diabetes, hypertension, wounds, infections, and the need for dental referrals. We address acute needs immediately and create a long-term care plan. We accept Medicaid (the most common coverage for this population) and can help uninsured patients explore coverage options through SC Healthy Connections.
Substance Use Treatment
Hope Addiction Medicine provides medication-assisted treatment for individuals in recovery housing. Sublocade monthly injection is especially valuable for this population — it eliminates daily medication compliance challenges and removes diversion concerns in shared living environments. One office visit per month provides 30 days of steady buprenorphine. We coordinate with Oxford House managers and sober living staff to support treatment adherence.
Behavioral Health
Hope Behavioral Medicine provides psychiatric medication management and counseling for anxiety, depression, and PTSD — conditions that are highly prevalent among individuals experiencing or recovering from homelessness. We understand the trauma of housing instability and provide care without judgment. Our providers are experienced with the complex social situations that accompany homelessness and housing transition.
For Shelter Staff & Housing Coordinators
Call (843) 737-0312 and ask for our referral coordinator. We can schedule group intake appointments for shelter residents to reduce individual scheduling burden. Our West Ashley office at 1064 Gardner Road is accessible via CARTA bus routes. We accept Medicare, most major commercial plans, and all South Carolina Medicaid managed care plans except Select Health (First Choice). Self-pay arrangements are available for uninsured patients.
