One Location, Four Practices: How Coordinated Care Works at Hope Health Network

Primary care, behavioral medicine and medication-assisted treatment under one roof in West Ashley, and what that means for patients and the people who refer them.

Healthcare in the Charleston Lowcountry is fragmented. A patient with diabetes, depression and an opioid use disorder can end up with three providers in three buildings who have never spoken to one another. Hope Health Network was formed to fix that for the people who need it most: four practices, one address, one care team.

Four practices, one location

  • Hope Medical Associates provides primary care for adults: annual physicals, chronic disease management and preventive care.
  • Hope Behavioral Medicine provides psychiatric medication management and counseling for anxiety, depression, ADHD and insomnia, in person and by telehealth across South Carolina.
  • Hope Integrative Medicine brings primary care, mental health and addiction medicine together for patients who want everything in one place with one chart.
  • Hope Addiction Medicine provides medication-assisted treatment for opioid and alcohol use disorders, including Suboxone, Sublocade and Vivitrol.

All four operate from 1064 Gardner Road, Suite 215 in West Ashley, Charleston, and share a Medical Director.

Why separate practices

Each practice focuses on one area of medicine so patients get specialists rather than generalists, and so each practice can be found by the people searching for exactly that kind of care. Keeping the practices distinct also lets a patient use one service without their other records being visible to anyone who does not need them.

What coordination looks like day to day

When a primary care patient needs help with mood or sleep, the referral goes down the hall rather than across town, and the first appointment is often within a week. When a patient in medication-assisted treatment needs a blood pressure medication adjusted or a hepatitis C screening, that happens in the same building. Providers share information only with the patient's consent and only as much as the other practice needs.

One insurance list, one phone number

All four practices accept the same plans: Medicare, most major commercial plans and all South Carolina Medicaid managed care plans except Select Health. Self-pay rates are available. One phone number reaches intake for every practice, and staff route the call to the right team.

Referring a patient

Community organizations, hospitals, courts, employers and faith communities refer patients to the network every week. Call during business hours or fax referral documentation, and our intake team will typically confirm the first appointment within two business days.
